Standards of medical care require that I keep a close eye on 
your medical condition so we request that you have regular 
visits for the following medical problems:

 

  • Diabetes Mellitus: You should been seen every 3 months for blood work and a check up on your blood sugar.
     
  • Hypertension/High Blood Pressure: You should be seen every 6 months for a blood pressure check, evaluation of medications and possible blood work.
     
  • Asthma: You should be checked every 6 months.
     
  • Birth Control: You must come in every 12 months for a pap smear.
     
  • Hyperlipidemia/Cholesterol: You must be seen and have fasting blood work every 6 months. If you have a medication dose change, check-ups are more frequent.
     
  • Physicals and Pap Smears: These appointments are not urgent and may be scheduled at least 2 weeks from the date you call for an appointment.
     
  • Medication Refills: We would like at least a 3 day notice for all refill requests. For all standard refill requests please contact  your pharmacy and have them fax over the refill request from. For mail order requests please allow additional time. Medications will not be filled on the weekends.
     
  • Referrals: Our office will process direct referrals within 72 hours. Once you are referred, you will then receive a letter of authorization from your medical group with your referral number and the physician’s information to contact for the appointment. The standard wait time to receive your letter of confirmation is approximately 7-10 business days.
       
  • Prior Authorization: For out of network provider requests, durable medical equipment, special radiology tests (such as an MRI) and certain procedures will require additional time. The wait time for authorization may take up to 14 business days from the medical group.
     
  • Urgent Referrals: Urgent referrals typically take 3 business days to be approved by the medical group/medical director.
     
  • Walk-in and Urgent Appointments: We do not take walk-in appointments, but we will do our best to see you within 24 hours for urgent visits.
     
  • Cancellations: We strive to offer our patients timely appointments, therefore we instituted the following office policies so that we can minimize our waiting time for appointments. Time slots are valuable so please give us a much notice as possible when canceling your appointment.
     
  • No Shows: You must contact our office at least 24 hours in advance to cancel your appointment. Less than 24 hour cancellation and not show anointments will be assessed a $25 fee. If it is your 3rd no show/less than 24 hours cancellation, you will be charged a $35 fee and 4 or more no show/less than 24 hours cancellation will be charged a $50 fee.
     
  • After Hours Care: After hours there is an on call service available for medical emergencies only. By calling our office you will be directed to the answering service. which will then contact the on call physician. Routine medication. birth control. or narcotics will not be prescribed or refilled over the phone by the on call physicians.
      
  • Insurance and Billing: All services including co-payments must be paid at the time of the visit. For your convenience The office accepts cash, check, visa. and mastercard. We do not do any third party billing. If you receive a bill from the laboratory, please contact the number on the bill to speak to them directly. If you receive a bill from our office, please contact the number on the bill so you may speak to a billing specialist.
      
  • Bad Check Fee: There is a $25.00 feel for all returned checks. After 1 returned check, you will only be able to make your payments using cash or credit card.
